4. Your name, mailing address, telephone number, and email address
5. A statement that you have a good faith belief that the use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law

Incomplete notices may not receive a timely or substantive response. We reserve the right to request additional information before processing any DMCA claim.
2. Our Response to Valid DMCA Notices
Upon receipt of a valid DMCA notice, we will:

We reserve the right to restore removed content if we receive a valid counter-notification or determine in our sole discretion that the original notice was invalid.

2. Identification of the material that has been removed or to which access has been disabled, and the location at which the material appeared before it was removed or disabled

Upon receipt of a valid counter-notification, we will forward it to the original complainant and inform them that we may restore the removed content in 10 to 14 business days unless the complainant notifies us that they have filed a court action against you.
4. Repeat Infringer Policy
In accordance with the DMCA and other applicable law, we have adopted a policy of terminating, in appropriate circumstances and at our sole discretion, users who are deemed to be repeat infringers. We may also limit access to the Site or take other action against any user who infringes the intellectual property rights of others, regardless of whether repeat infringement has occurred.
5. Misrepresentation Warning
